Environmental Modification Services-- Environmental
modifications cover those physical adaptations to the home, required
by the recipient's plan of care, which are necessary to ensure safety,
security, and accessibility of the individual and enables the individual
to function with greater independence in the home and without which,
the recipient MIGHT OTHERWISE require institutionalization.
Such modifications may include the installation
of ramps and
grab-bars, widening of doorways, modification of bathroom facilities,
or
installation of specialized electric and plumbing systems which
are necessary to
accommodate the medical equipment and supplies which are necessary
for the
welfare of the recipient.
These modifications shall exclude those adaptations
or improvements to the home which are not of direct medical or remedial
benefit to the waiver recipient, such as carpeting, roof repair,
central air conditioning, etc.
All services shall be provided in accordance with
applicable state and local
building codes.
Environmental modifications consist of
the following physical modifications of a principal place of residence
for the D&E waiver recipient:
1. Building and installation of ramps;
2. Installation of grab-bars;
3. Widening of doorways to accommodate wheelchair accessibility;
4. Modification of bathroom facilities;
5. Installation of specialized electric and plumbing systems necessary
to accommodate the medical equipment and supplies that assure the
health and welfare of the waiver recipient;
6. Other modifications that are necessary to the health and safety
of
the recipient and as approved by the Department.
NOTE: CONSULTATION WITH AND APPROVAL BY THE
DISABLED & ELDERLY MEDICAID WAIVER PROGRAM IS
NECESSARY WHEN THE PROJECTED COST OF SERVICE
EXCEEDS $2,000.00 PER THE PLAN OF CARE ELIGIBILITY
PERIOD.
Environmental modification services must be provided in accordance
with the
LTC&RSD Standards Document.
